Silver Haven Home Health LLC is seeking a Part-Time Licensed Vocational Nurse (LVN) to provide skilled nursing follow-up visits to home health patients throughout the Dallas area. This position is ideal for an LVN with home health experience who is looking for a flexible, patient-based schedule.
Duties
- Complete skilled nursing follow-up visits in patients’ homes.
- Monitor patient condition and report changes to the supervising RN.
- Provide medication management and patient/caregiver education.
- Perform wound care and other ordered skilled nursing treatments within LVN scope.
- Follow established plans of care and physician orders.
- Complete accurate and timely visit documentation.
- Communicate and coordinate with the RN, DON, physicians, and care team.
Requirements
- Current and unrestricted Texas LVN license.
- Home health experience preferred.
- Current CPR/BLS certification.
- Strong clinical assessment and documentation skills.
- Valid driver’s license, auto insurance, and reliable transportation.
- Ability to travel throughout the Dallas service area.
- Ability to work independently and complete documentation timely.
