Home Health Weekend Registered Nurse

Overview HCA Healthcare is seeking a compassionate and reliable Home Health Weekend Registered Nurse to provide high‑quality, patient‑centered care to individuals in their homes. As part of our Healthcare & Medical Services team, you will deliver skilled nursing services to patients who require ongoing clinical support outside the hospital setting, helping them manage acute and chronic conditions safely and comfortably at home. Responsibilities Working primarily on weekends, you will conduct comprehensive patient assessments, develop and implement individualized care plans, administer medications and treatments, and coordinate care with physicians, therapists, and other members of the interdisciplinary team. In this role you will travel to patient residences within the service area to perform visits, monitor patient progress, and adjust care plans based on clinical findings and physician orders. You will provide hands‑on clinical care such as wound care, IV therapies within scope, catheter care, and disease‑specific management education for patients and caregivers. Accurate and timely documentation in the electronic medical record is essential, as you will be responsible for recording assessments, interventions, patient responses, and communication with providers in accordance with regulatory and HCA Healthcare standards. The Home Health Weekend Registered Nurse plays a key role in promoting patient safety, preventing hospital readmissions, and supporting quality outcomes. You will educate patients and families on medication management, symptom recognition, equipment use, and lifestyle modifications to support recovery and long‑term health. You will also participate in care coordination, collaborating closely with physicians, home health aides, therapists, and other clinicians to ensure continuity of care and a positive patient experience.
