Healthcare Outreach Coordinator (Part-Time)

Location: Huntsville, AL (Madison & Limestone Counties) About the Role Reach Angels Home Care is seeking a Healthcare Outreach Coordinator to build referral relationships and support Veterans in accessing home care services. This is a field-based role focused on visiting facilities, building partnerships, and generating referrals. What You’ll Do Build and maintain referral relationships Present our services (same-day start of care available) Follow up consistently with partners Generate and track referrals Requirements Experience in healthcare (home care, hospice, hospital, or medical sales) Comfortable with in-person outreach daily Strong communication and professionalism Preferred Existing relationships with VA, hospitals, or ALFs Experience working with referrals or patient coordination Compensation Bonus per referral that starts care Opportunity to grow into full-time Qualifications Strong Communication and Customer Service skills to effectively interact with clients, healthcare professionals, and community partners. Experience in Community Outreach and relationship-building to foster and maintain connections with local organizations. Proficiency in Research to identify community needs and opportunities for engagement. Ability to support and conduct Training sessions for staff and community partners as needed. Excellent organizational and time-management skills for coordinating outreach activities. Knowledge of local healthcare and community resources is highly beneficial. Bachelor's degree in Healthcare Administration, Social Work, Public Health, or a related field is preferred but not required.
