Nurse Supervisor, Care Management

A New York City hospital is seeking an experienced registered nurse to lead its Care Management department in a supervisory capacity. This position carries Associate Director-level responsibility and is well suited to a clinician with a strong foundation in discharge planning and utilization review who is ready to step into a department leadership role. The team's focus is on coordinating patient care transitions and managing clinical resource utilization to achieve strong patient outcomes.

Schedule: Monday through Friday, 9:00 AM to 5:30 PM, with some weekend coverage required.

Compensation: $115,000 to $140,000 annually

What You'll Do:

You will provide Associate Director-level supervision to the Care Management department, overseeing day-to-day operations and the clinical performance of the team. That includes guiding staff on complex discharge planning cases, conducting and overseeing utilization review activities, and ensuring the department meets quality, compliance, and patient care standards.

You will serve as a clinical resource and decision-maker for care management staff, supporting them through escalated cases and providing direct mentorship. You will also collaborate with interdisciplinary teams across the organization to facilitate timely and appropriate care transitions, coordinate with payers and external providers as needed, and contribute to departmental planning and reporting.

Required Qualifications

• Active Registered Nurse (RN) licensure in New York State
• Minimum 2 years of experience in discharge planning and utilization review
• Active Certified Case Manager (CCM) certification
• Active Patient Review Instrument (PRI) certification
• Bachelor of Science in Nursing (BSN) or related field

Preferred Qualifications

• Master's degree in Nursing, Health Administration, or a related field
• Prior supervisory or department leadership experience in a care management or case management setting
• Experience working with payers, managed care organizations, or value-based care programs
• Familiarity with InterQual or Milliman clinical criteria
