LVN/LPN Nursing Position

Duration: 13 weeks Shift: M-F Hours per Shift: 8:00 AM - 4:30 PM Experience: Minimum of one (1) year of nursing experience in a clinical setting with a frail or elderly population. License: Unencumbered LVN/LPN License Certifications: Nursing knowledge and skills necessary to treat frail, elderly participants and manage complex clinical situations.

Description: Under the direction of the Provider and Clinical Nurse Manager, the LVN/LPN is responsible for providing nursing care to participants in accordance with their scope of practice both inside and outside of the clinic setting. The LVN/LPN will collaborate with the clinical team to facilitate the efficiency of care provided. Responsibilities include wound care, point-of-care testing, medication administration, and coordination support. The LVN/LPN will also provide care coordination support tasks outside of the clinic setting within the scope of practice. The LVN/LPN will act as a liaison between participants, their family members, the provider, nurse case manager, and other members of the support network, as appropriate. Collaboration with colleagues and stakeholders is essential to promote values and team culture.

Responsibilities include performing and documenting ordered wound care in the clinic setting and in accordance with the scope of practice. The LVN/LPN will perform and document ordered procedures and care in the clinic, including medication management and administration. The LVN/LPN will administer ordered medications to participants while in the clinic and reinforce education regarding medications. The role includes processing Specialty Pharmacy medication orders and addressing medication issues and questions within scope. The LVN/LPN will coordinate with the provider on any medication order changes stemming from same-day procedures. Review and reconcile medications during Welbe Welcome Day for new participant visits is a key responsibility. Monthly inventory of medications, if applicable, is also required. The LVN/LPN will perform and document pre and post-operative clinical touchpoints, reviewing any instructions provided by the specialist office. High-risk monitoring of participants for vitals and medication reminders is necessary. The LVN/LPN will document proactive quality participant touchpoints and escalate questions or concerns as appropriate. It is important to remain alert to input from team members, participants, and caregivers and update the Clinical Nurse Manager and Provider promptly of any changes in participant conditions. Legally authorized to perform in the position location is required. The LVN/LPN must act within the scope of authority to perform duties and complete required documentation in a timely and accurate manner. Privacy and confidentiality of all procedures and information is essential. Following policies and procedures and participating in required training is expected. Regular communication with the Supervisor and team regarding workload and priorities is important. Timeliness in completing mandated training and occupational health screenings is required. Exercise flexibility in assignments as business needs evolve. Additional duties may be assigned as necessary.
