Optometrist - Health Equity for Adolescents in the Legal System (HEAL) - Per Diem

Position: Optometrist (Per Diem) – Physician Affiliate Group of New York (PAGNY), NYC Health + Hospitals/Administration for Children's Services. Per Diem, Department: Allied Health. Location: Health Equity for Adolescents in the Legal System, Brooklyn, New York. Hourly rate: $100. Job Description The HEAL Program is a new initiative to transition into being the primary provider of medical services to young people in city custody. The staff will be responsible for 24/7 medical care across two secure detention sites, two school-based health centers, and limited pharmacy services at seven non‑secure sites (group homes). Services include comprehensive medical screenings, dental screenings and procedures, vaccinations and routine testing, responses to immediate needs, and referrals for off‑site care within and outside the NYC Health + Hospitals system. Responsibilities Routine on‑site eye examinations, including problem‑oriented history, visual acuity, eye health assessment, refraction, disposition, and eyeglass order generation. Completion of documentation, records, and any other vision services within the equipment parameters and scope of Optometrists as provided by the State. Referral to an ophthalmologist, other physician(s), or telemedicine services for care beyond the scope of on‑site Optometry, as needed. Qualifications Doctor of Optometry (O.D.)
