Nurse Practitioner / Physician Assistant, Internal Medicine Transplant Nephrology

The University of Cincinnati College of Medicine, in partnership with the UC Health academic healthcare system and Cincinnati Children's Hospital Medical Center, is seeking a full-time Nurse Practitioner or Physician Assistant for the Department of Internal Medicine, Division of Nephrology and Hypertension. This position will work in the transplant section within the division of Nephrology to provide inpatient, outpatient, pre- and post-transplant services at UCMC. This position will enhance the quality of care given to transplant patients by providing dedicated care to non-renal solid organ transplants, including long-term transplants.

Essential Functions:

• Practice clinical/patient care according to professional practice standards and institutional policy/procedure.
• Ongoing assessment, when appropriate, is performed and documented according to policy.
• Interpretation of the patient's condition and abnormal findings is communicated in a timely manner to the physician and all appropriate members of the interdisciplinary healthcare team.
• Participating in hospital and provider performance improvement activities.
• Caring for all aspects of outpatient functions for kidney transplant clinic including waitlist management.
• Responsible for coordination of care for transplant patients across their various multi-disciplinary teams ensuring necessary patient care is accomplished.
• Education for transplant patients' including medication adherence, signs of rejection/infection, vaccination guidelines, lifestyle modifications, transplant process, and living donation.
• Provide cross-coverage for fellow transplant APPs during scheduled time off, managing their patient panels for routine and urgent needs to ensure continuity of care.
• Perform related duties based on departmental need.

Nurse Practitioner Required Education: Master's Degree in Nursing from an accredited nursing program. Required Trainings/Certifications: Active Ohio APRN license with prescriptive authority. Active Board Certification in appropriate nursing specialty. Required Experience: Three (3) years nursing experience. - OR - Physician Assistant Required Education: Master's Degree from an accredited Physician Assistant program. Required Trainings/Certifications: Active Ohio Physician Assistant license with prescriptive authority. Certification as a Physician Assistant by the National Commission on Certification of Physician Assistants.
