Home Health Physical Therapist Assistant

Addus Family Company is seeking a compassionate Physical Therapist Assistant to support in-home rehabilitation services for older adults and individuals with chronic or disabling conditions. Working under the direction of a licensed Physical Therapist, you’ll implement treatment plans, guide patients through therapeutic exercises, monitor progress, and document outcomes in accordance with regulatory and company standards. You’ll collaborate closely with caregivers and the clinical team, educate patients and families on safe mobility and home exercise programs, and help clients maintain independence and quality of life in their own homes.
