About the Position

Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience, depending on the specialty. License: Must have license in hand, no exceptions. Certifications: CPR, BLS Must-Have: - Proficiency in HomeCare HomeBase EMR and Oasis, used within the last 6 months. - Must have reliable transportation, a current driver's license, and agency-required liability insurance. - Must be willing to travel up to 75 miles from the assigned care center. Description: Provides coordinated and directed home nursing care according to physician's orders. Responsible for adhering to agency policies and procedures through the application of the nursing process. Oversees and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Conduct home visits and participate in periodic office meetings. Average number of patients per day ranges from 5 to 10. Documentation and travel activities are part of the daily responsibilities. Types of cases include Start of Care, Wound Care, and Follow-Up Visits. On-call responsibilities may vary and include being on-call once a month for one week, including the weekend. All documentation must be completed within 24 hours of the patient appointment. Delayed or incomplete documentation may impact billing and follow-up patient care. Contract hours are not guaranteed; clinicians must meet weekly productivity expectations of a minimum of 30 points. Declining patients may result in cancellation of the contract. Local candidates are accepted at the same rate. Traveler candidates must not currently be direct employees of any facility. Candidates must understand and accept the travel and schedule demands required for this position.
