Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum 1 full year of Adult Home Health experience or 1 full year of Hospice experience depending on the specialty. License: Must have an active nursing license in hand. Certifications: CPR, BLS Must-Have: Proficiency in OASIS and HomeCare HomeBase (HCHB) within the last 6 months. Ability to travel up to 75 miles to patient homes. Reliable transportation, a current driver’s license, and agency-required liability insurance. Comfort with longer drives between patients as this role requires daily travel to various locations. Description: Provides, coordinates, and directs the provision of home nursing care according to physician's orders based on agency policies and procedures. Responsible for the competent application of the nursing process. Supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Conducts home visits and attends periodic office meetings. The contractor will be assigned between 5 to 10 patients per day. This equates to roughly 6 to 8 productivity points per day. Types of cases include Start of Care, Wound Care, and Follow-up Visits. Documentation must be completed within 24 hours of patient appointments. All travel is compensated from patient home to patient home, excluding the commute to and from the first and last patient. Must accept every patient on their schedule regardless of distance from previous appointments. On-call expectations may vary, typically requiring 2 to 4 times a month. Traveling may result in daily mileage of 20 to 40 miles. Weekly productivity expectations are set at 30 points. All travelers must have recent and relevant specialty experience.
