What You'll Do

As a Home Health LPN, you will provide skilled nursing care under the direction of a Registered Nurse and physician while supporting patients and families throughout their healthcare journey.

**Responsibilities include:**

* Delivering care in accordance with physician orders, delegated nursing responsibilities, and the established Plan of Care* Monitoring patient conditions and reporting significant changes to the RN Case Manager and healthcare team* Assisting patients and caregivers with education and self-care techniques* Implementing patient safety plans and utilizing available community resources* Collaborating with the interdisciplinary care team to ensure exceptional patient outcomes* Documenting clinical findings, nursing interventions, and medication updates accurately and timely* Identifying opportunities to improve patient outcomes and reduce avoidable hospitalizations and emergency room visits* Providing compassionate, professional support to patients, families, physicians, and colleagues

Qualifications

* Graduate of an accredited Certificate, Diploma, or Associate Degree Nursing program* Current Massachusetts Licensed Practical Nurse (LPN) license* Minimum of one (1) year of clinical nursing experience* Valid driver's license, reliable transportation, and current auto insurance* Ability and willingness to travel throughout the assigned territory

**Preferred:**

* Previous home health or hospice experience

* Physical Requirements - Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
