Clinical Pharmacist

Overview This Staff Pharmacist position primarily works weekdays with one Saturday coverage per month and supports our Long-Term Care pharmacy serving the Oregon PACE program and Providence Child Center. The pharmacist ensures safe, effective, high-quality medication therapy for geriatric and pediatric patients in a facility-based care environment. Key Responsibilities Interpret, verify, and process physician medication orders with accuracy and timeliness. Monitor drug therapy to ensure safety, appropriateness, and optimal clinical outcomes. Provide pharmaceutical care for long-term care patients, including geriatric and pediatric populations. Collaborate closely with physicians, nurses, clinical pharmacists, and care teams within the Oregon PACE program and Providence Child Center. Ensure compliance with all professional standards, regulatory requirements, and pharmacy best practices. Support and manage cycle fill processes, Medication Administration Records (MARs), and facility-based medication systems. Provide clinical recommendations and drug information to healthcare providers. Perform medication reconciliation during routine cycle fills. Required Qualifications Education that meets certification, license, or registration requirements. Upon hire: Oregon Pharmacist License. One year of pharmacist experience or completion of a PGY-1 residency preferred. Preferred Qualifications Previous experience in Long-Term Care, PACE programs, institutional pharmacy, or retail pharmacy settings. Board certification or advanced clinical training. Experience working in interdisciplinary healthcare teams. Familiarity with electronic health records and pharmacy systems used in LTC settings. Comfortable working in a high-volume prescription environment. Benefits Pay range: $72.42 – $114.33 per hour, with potential for shift differentials, standby/on-call pay, overtime, premiums, extra shift incentives, or bonus opportunities.
