In this role, you will provide individualized nursing care, manage a dedicated caseload, and help patients recover, manage chronic conditions, and maintain their independence at home.\n \n Home health nursing gives you the opportunity to slow down, focus on one patient at a time, and make clinical decisions based on what each patient truly needs—all while having the autonomy to manage your day with the support of an experienced interdisciplinary team.\n Why Join Elara Caring?\n \n \n One-on-one patient care\n \n Autonomy and flexibility in managing your schedule\n \n Supportive and collaborative clinical team\n \n Competitive compensation (pay is based off experience and education with a range $88-105K)\n \n Comprehensive onboarding and ongoing clinical support\n \n Medical, dental, and vision insurance\n \n 401(k) with employer match\n \n Paid time off and paid holidays\n \n Tuition reimbursement for full-time employees\n \n Continuing education opportunities for all employees\n \n Family and pet bereavement leave\n \n Pet insurance\n \n Opportunities for professional growth and advancement\n \n What You’ll Do\n As a Home Health Registered Nurse, you will take primary responsibility for coordinating and managing an assigned patient caseload.\n Your responsibilities will include:\n \n \n Completing comprehensive initial and ongoing nursing assessments in patients’ homes\n \n Evaluating each patient’s physical, psychosocial, environmental, and safety needs\n \n Developing, implementing, coordinating, and updating individualized Plans of Care\n \n Providing skilled nursing services in accordance with physician orders, agency standards, and applicable regulations\n \n Monitoring patients’ conditions, progress, medications, symptoms, and response to treatment\n \n Performing follow-up visits based on each patient’s clinical needs and Plan of Care\n \n Educating patients and caregivers on diagnoses, medications, treatments, disease management, self-care techniques, and signs that require clinical attention\n \n Coordinating care with physicians, therapists, social workers, home health aides, facilities, and other members of the interdisciplinary team\n \n Communicating significant findings, changes in condition, safety concerns, or environmental risks promptly to the appropriate clinical personnel\n \n Revising the Plan of Care when patient needs change and obtaining updated physician orders as required\n \n Helping patients and caregivers access appropriate clinical, family, and community resources\n \n Taking proactive steps to reduce avoidable hospitalizations and emergency department visits\n \n Maintaining timely, accurate, and complete clinical documentation\n \n Protecting patient and employee privacy and confidentiality in accordance with HIPAA requirements\n \n Participating in quality-improvement, safety, and patient-outcome initiatives\n \n Performing additional duties and special projects as assigned\n \n What You’ll Need\n \n \n Graduate of an accredited nursing program\n \n Current, unrestricted Registered Nurse license in the applicable state\n \n At least one year of nursing experience in a clinical care setting\n \n Associate Degree in Nursing required; Bachelor of Science in Nursing preferred\n \n Previous home health or hospice experience preferred\n \n Homecare Homebase experience preferred\n \n Strong clinical assessment, communication, organization, and care-coordination skills\n \n Ability to manage an assigned caseload and work independently in the field\n \n Ability to collaborate effectively with patients, caregivers, physicians, and an interdisciplinary clinical team\n \n Dependable vehicle, valid driver’s license, and current automobile insurance in accordance with state requirements\n \n Willingness and ability to travel within the assigned branch coverage area\n \n Sit, stand, bend, lift and move intermittently and be able to lift 50–100 lbs.\n \n \n \n If you are ready to bring your nursing skills into a setting where you can focus on one patient at a time and see the real-life impact of your care, we would love to connect with you.
