What You Will Do
As a Physical Therapist in our Skilled Nursing setting, you will evaluate and treat patients recovering from orthopedic surgeries, neurological events, cardiac conditions, and general deconditioning. You will develop individualized treatment plans, collaborate with the interdisciplinary care team, and play a direct role in helping patients return to their highest level of independence. Your caseload will be manageable, and your clinical judgment will be respected.
You will conduct comprehensive occupational therapy evaluations, develop individualized treatment plans focused on functional independence, and collaborate with nursing, PT, SLP, and social work to ensure coordinated patient care. You will address self-care skills, cognitive and perceptual deficits, home safety, adaptive equipment needs, and caregiver training. Your clinical reasoning will be valued, and your productivity expectations will be realistic.

What You Bring
- Doctor of Physical Therapy (DPT) or Master's degree from an accredited PT program/ Master's or Doctoral degree in Occupational Therapy from an accredited program
- Current or eligible for New York State PT/ OT license
- Passion for geriatric rehabilitation and functional outcomes
- Strong communication and teamwork skills
- New graduates are encouraged to apply we provide structured mentorship
