Position Overview

Seeking an experienced Registered Nurse (RN) to join our team serving patients throughout Rutherford County, TN . This role is responsible for providing skilled nursing care, completing comprehensive patient assessments, developing and implementing individualized care plans, and delivering compassionate, high-quality care in the home setting. The ideal candidate will have strong clinical judgment, excellent communication skills, and a commitment to improving patient outcomes through education, coordination, and continuity of care.

Schedule & Work Details

• Location: Rutherford County, TN
• Schedule: Monday-Friday, 8:00 AM - 4:30 PM
• Productivity Expectation: 30 points per week
• On-Call Requirement: Minimum of 1 weekend per month with LPN backup support
• Mileage Reimbursement: Yes, reimbursed between patient visits
• Pay Rate: $51-$53/hour

Key Responsibilities

• Perform comprehensive patient assessments and develop individualized nursing care plans.
• Provide skilled nursing care, treatment, rehabilitation support, and disease management in the home setting.
• Identify and address patients' physical, psychosocial, safety, and comfort needs.
• Administer medications, treatments, and nursing interventions within scope of practice.
• Monitor patient progress, evaluate outcomes, and update care plans as needed.
• Observe, document, and communicate changes in patient condition to physicians and clinical leadership.
• Educate patients, families, and caregivers on health maintenance, disease management, and treatment plans.
• Maintain accurate and timely clinical documentation in accordance with agency, state, and regulatory requirements.
• Collaborate with physicians, therapists, and interdisciplinary team members to ensure coordinated patient care.
• Supervise and support auxiliary nursing personnel and promote adherence to the nursing process and best practices.

Required Qualifications

• Active Tennessee Registered Nurse (RN) License
• Current BLS Certification
• Minimum 3 years of RN experience
• Minimum 1 year of Home Health RN experience
• Strong clinical assessment and care planning skills
• Valid driver's license and reliable transportation
• Ability to work independently while managing a home-based patient caseload

• Experience with home health documentation systems and electronic medical records (EMR)
• Strong knowledge of regulatory and compliance standards
• Excellent patient education and communication skills
• Proven ability to prioritize tasks and manage a dynamic schedule
