Overview Physical Therapy Assistant – LTAC, Hampton, NH, United States Start date: 08/17/2026 for a 13-week travel assignment. Weekly pay: $1,482–$1,511 per week with guaranteed hours of 35. Shift: 5x8 Days. Setting: LTC - Assisted Living Facility. This travel role is with HealthforceHealthcare LLC and is classified as Temporary Assignment. Key Highlights - Weekly pay: $1,482–$1,511 per week - Guaranteed hours: 35 per week - Contract duration: 13 weeks - Shift: 5x8 Days (08:00–16:30) - Start date: 08/17/2026 - Setting: Long-term care / assisted living environment Responsibilities - Implement and progress PT plans for active patients under supervision, focusing on functional mobility, strength, and balance. - Assist with therapeutic exercises, gait training, transfers, and activities of daily living training. - Prepare and position patients for therapy sessions; monitor patient response and report changes to the supervising PT. - Document treatment provided, progress notes, and any deviations from the plan of care in the medical record. - Maintain a safe, clean, and organized therapy area; ensure equipment is in good working order. - Collaborate with an interdisciplinary team to support discharge planning and patient goals.
