Registered Nurse Case Manager (RN CM)

Job Description

We are seeking a dedicated and professional Registered Nurse Case Manager to join our team. The ideal candidate will possess strong case management skills and experience in the field, particularly with inpatient care management. This position offers a structured day shift schedule, allowing for work-life balance.

Schedule

Day Shift: 8:00 AM - 4:30 PM
Days: Sunday through Thursday
Hours: 40 hours per week

Key Responsibilities

Manage a diverse and heavy caseload of patients.
Provide comprehensive care management services in accordance with established protocols.
Collaborate with multidisciplinary teams to optimize patient outcomes.
Utilize case management tools and resources to assess patient needs and develop individualized care plans.
Ensure compliance with regulatory standards and organizational policies.

Qualifications

Education: Bachelor's Degree in Nursing (BSN) preferred.
Experience: Prior experience in a care management role preferred; inpatient case management experience highly preferred.
Licenses/Certifications:
- Registered Nurse (RN) licensure in the state of practice: Required.
- Case Management Certification: Preferred.
- Basic Life Support (BLS) certification from an approved vendor per policy: Required.

Preferred Qualifications

Familiarity with strike force operations for healthcare systems.
Ability to adapt quickly and manage a fast-paced work environment with minimal training.
