TRAVEL LICENSED PRACTICAL NURSE (LPN) JOB OVERVIEW

Seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide quality nursing care in a long-term care and memory care environment. The LPN will work closely with residents, families, and interdisciplinary healthcare teams to ensure patient safety, comfort, and well-being while delivering exceptional nursing services.

SHIFT DETAILS

• Shift Time: 2:00 PM 10:30 PM
• Meal Break: 30 Minutes
• Evening Shift
• Full-Time Rotation Schedule
• 4 Days On / 2 Days Off Rotation

GUARANTEED HOURS & PAY

• Guaranteed Hours: 40 Hours Per Week
• Pay Rate: $30.90 - $36.05 per hour

REQUIREMENTS

• Active Licensed Practical Nurse (LPN) License in good standing
• Current BLS certification preferred
• Previous long-term care, skilled nursing, memory care, or rehabilitation experience preferred
• Strong clinical assessment and documentation skills
• Excellent communication and interpersonal abilities
• Ability to work effectively within an interdisciplinary healthcare team

KEY RESPONSIBILITIES

• Provide direct nursing care and treatment to residents
• Administer medications and treatments as prescribed
• Monitor resident conditions and report changes appropriately
• Maintain accurate and timely clinical documentation
• Collaborate with physicians, nurses, and other healthcare professionals
• Assist in implementing individualized care plans
• Educate residents and families regarding care plans and treatment needs
• Promote resident safety, dignity, and quality of life
• Ensure compliance with facility policies and regulatory standards

ADDITIONAL INFORMATION

• Guaranteed 40 hours per week
• Competitive pay rate of $30.90 - $36.05 per hour
• Evening shift schedule
• Experience with memory care and dementia populations preferred
