What You’ll Do Travel to senior communities to provide patient care Perform wellness checks, chronic care, and wound assessments Coordinate care plans with nursing and family members Chart in EMR and ensure regulatory documentation Qualifications Licensed Nurse Practitioner (AGNP or CNP) Experience with wound care preferred (certification paid) Passion for elder care Self-motivated with strong communication skills Why Join TCP?
