Experience: Prior experience in Long Term Care setting preferred

What You'll Do

• Screen, evaluate, and treat patients with physical functioning disorders or wounds, following clinical standards.
• Utilize standardized assessments and evidence-based practices to support treatment plans.
• Administer rehabilitation procedures including manual techniques, ambulation, exercises, modalities, and wound care.
• Monitor patient progress, adjusting treatments based on outcomes and referrals.
• Communicate effectively with the healthcare team, patients, families, and caregivers.
• Document all services and treatment outcomes accurately in electronic records.
• Support and supervise rehabilitation aides, students, or assistants as needed.
• Participate in meetings and training sessions as required.

What You Need

Required:

• Valid Massachusetts Physical Therapist license
• Experience in Long Term Care or Rehabilitation preferred
• Knowledge of clinical practices and documentation requirements
• Strong communication skills and patient-centered approach

Preferred:

• Experience with wound care and manual therapy techniques
• Ability to work collaboratively in a multidisciplinary team
