Nurse (Mental Health Care Facilitator)

Job Description

Perform independent, comprehensive telephone and clinical triaging in the primary care setting, supporting primary care providers in their treatment of patients with mental health conditions.

Monitor patient progress and symptom severity by conducting a structured telephonic interview and using validated tools.

Communicate with providers regarding patient status and documents appropriately in the electronic medical record.

Coordinate patient referrals to support services provided through the internal and external community, or the TRICARE network.

Compiles and maintains a variety of data reports, submitting detailed and aggregate program and caseload data reports as needed by program staff.
