Social Worker

Join our team at Cape Cod Post Acute as a Social Worker! Proudly supported by Marquis Health Consulting Services. Full-time 40 hours; $25-$50 an hour . Responsibilities Evaluate patients (existing, new and prospective) based on their needs and limitations. Address patient concerns and goals while maintaining constant communication with the patient as well as a case manager. Build rapport with patients and their families. Coordinate care with PT departments. Develop care plans for patients. Qualifications Must be able to work full-time hours and be flexible. Bachelor’s degree required. Master’s degree is strongly preferred. Minimum three (1) years' experience as a social worker in a healthcare-related setting. Experience within a nursing home or rehabilitation center is strongly preferred. Knowledge of the rules, regulations, and guidelines that govern nursing care facilities. Communication skills with the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the public. LSW Required.
