Primary Care Physician

Role: Role: Primary Care Physician responsible for delivering equitable, value-based healthcare to Medicare patients in a neighborhood clinic setting. Responsibilities include: assessing and diagnosing patients, overseeing primary care services, prescribing medications, maintaining electronic medical records, collaborating with leadership to meet quality goals, managing care teams, and performing other duties as assigned. Qualifications: Qualifications: Medical degree (MD or DO), board certification or eligibility in Internal Medicine or Family Medicine, active unrestricted license, DEA license, and US work authorization. Preferred qualifications include geriatrics fellowship, experience with geriatric populations, value-based care, outpatient settings, and bilingual skills. Skills: Skills: strong clinical assessment, teamwork, care management, and documentation capabilities. The role emphasizes quality care, outcome-driven practices, and collaboration with care teams and leadership.
