Job Summary and Qualifications The Clinical Pharmacist is responsible for supporting the Director of Pharmacy in managing the pharmacy clinical operations. This includes implementation and enforcement of policies and procedures, supervision and training of staff, quality assurance and regulatory compliance. The Clinical Pharmacist shall also assist in managing the pharmacy when the Pharmacy Director is unavailable. PRINCIPAL DUTIES/RESPONSIBILITIES: Interprets orders and transcribes or verifies them accurately and efficiently into the pharmacy computer system. Accurately compounds and dispenses pharmaceuticals. Complies with facility policy and procedures and regulatory requirements. Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose. Suggests appropriate, cost-effective therapeutic medication alternatives to medical staff. Sustains the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges and promotes rational drug therapy selection. Provides accurate and timely medication information to healthcare staff and patients. Assists with clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs as defined by Pharmacy Leadership. Assists with the antimicrobial management and other clinical programs. Documents all clinical activities and interventions accurately and completely. Supports and participates in the goals of the organization's quality improvement, risk management and patient safety program to include but not limited to National Patient Safety Goals, Core Measures and HCAHPS medication related initiatives. Completes notification reports regarding medication errors and adverse drug reactions. Participates in the training of new pharmacists, pharmacy students, and technicians. Maintains professional competency sufficient to meet current and future needs of the facility. Has knowledge of the patient population served and applies that knowledge in the care of those patients. Provides basic technical support for automated dispensing machines and other pharmacy computer systems. Effectively supervises pharmacy clinical operation activities and provides support to the whole pharmacy team. Compliant with facility pharmacy schedule to work physically at patient care areas to interact with medical staff, nurses and other members of the healthcare team. Effectively and proactively intervenes in conflicts between pharmacy and medical staff regarding drug therapy issue resolutions. Trains and participates with Disaster Team drills and real-event response as assigned. Trains and participates with Code Teams as assigned. Obtain and maintain BLS-ACLS or PALS current certification as appropriate. Maintains appropriate controlled substance records. Works with the Pharmacy Director to identify and develop processes to meet or exceed all controlled substance diversion prevention and monitoring standards. Participates in controlled substance reviews and audits. Complies with all regulatory standards. Prioritizes work and leads others to comply with facility medication turn-around time policy and procedure. Identifies areas for improved efficiency. Assists in implementation of process changes. Supervises and assists pharmacy technicians and other support staff effectively. Works with the director to establish pharmacy goals and objectives and responsible for implementation and monitoring. Participates with various nursing and medical staff committees as necessary. Responsible in conjunction with pharmacy director and clinical manager for staff performance evaluations. Acts as manager in charge as assigned. Assists with the preparation of the pharmacy newsletter and/or other forms of written and electronic communication. Responsible for managing the drug formulary by insuring operational and inventory efficient processes are in place and followed. Responsible for the development, implementation, and monitoring of pharmacy operations and share clinical initiatives responsibility with Clinical Pharmacy Manager. Responsible for the development and review of pharmacy policies and procedures. Participates in the design and update of pharmacy computer system programs relating to pharmacy, nursing and physician programs. Participates in the maintenance of pharmacy dictionaries including adding new drugs to the drug dictionary and gold Standard formulary Charge Master maintenance. Oversight of staff schedules. Understands that failure to follow established SOP's for hazardous drugs could potentially lead to risk of adverse effects which may include but not limited to skin rashes, reproductive effects, chronic effects, and cancer. Performs other duties as assigned. What qualifications you will need: Education/Skills: B.S. or Pharm.D. from an ACPE-accredited School of Pharmacy. A Doctor of Pharmacy from an ACPE-accredited college of pharmacy is preferred. Experience: Minimum 1 year as a Clinical Pharmacist in a hospital setting or completion of a hospital PGY1/pharmacy specialty residency or fellowship. Licenses, Registrations and/or Certifications: Active pharmacist license in good standing with the appropriate State Board of Pharmacy ACPE accredited IV certification.
