Outreach Coordinator, Peace Hospital

Job Overview Primary Location: Peace - Louisville Address: 2020 Newburg Rd. Louisville, KY 40205. Shift: First Shift (United States of America). UofL Health is a fully integrated regional academic health system with multiple hospitals, medical centers, and a focus on delivering patient-centered care. Responsibilities Assess community behavioral health needs, provide input for facility strategic planning, and develop business and community outreach plans. Maintain ongoing communication with key facility referral sources to promote information about Peace’s services and provide customer service coordination of patient care. Provide Level of Care Assessments as needed and discuss treatment/recommendations with referral sources. Review referral source data and track trends; initiate face-to-face contact with referral sources throughout the assigned region. Maintain positive referral source relationships in targeted regions and process referral source complaints in collaboration with the Director Behavioral Health Outreach and administrative personnel. Participate in monthly department meetings and complete other outreach duties as assigned. Abide by the Standards of Conduct, state and federal regulatory requirements, and maintain a professional image in the workplace. Other Functions Abide by UofL Health policies regarding attendance and timeliness. Achieve excellence through teamwork to provide desired outcomes. Exhibit willingness to learn and perform tasks outside the regular scope of duties or work additional hours/shifts as necessary. Complete all assigned Annual Competencies and KNOW Modules by their due dates. Measure of Success No deficiencies in completion of assigned KNOW modules. Maintain compliance with all company policies, procedures, and standards of conduct. Comply with HIPAA privacy and security requirements to maintain confidentiality at all times. Perform other duties as assigned. Qualifications Education: Master’s degree in a human services field (required). Experience: At least 2 years’ experience in a behavioral health setting working independently in marketing, business development or clinical setting (required).
