As a Nurse Practitioner specializing in home health, you will conduct comprehensive risk assessments for Medicare Advantage members throughout Delaware County. This role offers autonomy and variety as you perform assessments that inform ongoing patient care without prescribing medications or developing treatment plans.

Schedule:

• Flexible shifts available any day of the week, Monday through Sunday
• Design your own schedule to fit your lifestyle
• Choose a minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform annual and post-discharge home health risk assessments in a variety of residential settings
• Review and document patients medical histories, existing conditions, and medications
• Provide education to patients about their health conditions, medications, and prevention
• Assess and report gaps in care, offering informed recommendations for improvement
• Engage with patients in locations throughout Delaware County, including the areas of Muncie, Yorktown, and Chesterfield

Preferred Qualifications:

• Active Nurse Practitioner license in Indiana
• Current BLS (Basic Life Support) CPR certification
• National Board Certification through AANP or ANCC
• Completed MSN from an accredited nursing program
• At least 6 months of current experience working as an NP in a clinical setting
