We're Hiring: Mds Coordinator

Are you an experienced Mds Coordinator who takes pride in accuracy, compliance, and quality resident care? We're looking for a knowledgeable, organized, and compassionate Mds Coordinator to join our team and play a key role in ensuring our residents receive the care they deserve.

What You'll Do

• Coordinate and oversee the Mds assessment process
• Ensure assessments are accurate, timely, and compliant with current Cms requirements
• Collaborate closely with Nursing, Therapy, Social Services, Dietary, and other interdisciplinary team members
• Lead and facilitate care plan development and coordination
• Monitor documentation and identify opportunities to improve clinical outcomes
• Support regulatory compliance and survey readiness
• Help ensure the interdisciplinary team is working together toward the best outcomes for our residents
• Maintain certs/recerts
• Initiate death certificates

What We're Looking For

• Previous Mds Coordinator experience in a Skilled Nursing Facility
• Strong knowledge of Mds 3.0 / Rai processes
• Understanding of Medicare, Medicaid, and Snf reimbursement requirements
• Excellent organization skills and attention to detail
• Strong communication and leadership skills
• A team player who is committed to resident-centered care
