Health Care Management Nurse Location: Oklahoma City, Oklahoma. Travel is occasional. Must possess a valid driver's license and maintain required car insurance. Annual Salary Level I: Y15A - $58,055.13 + Full State Employee Benefits Level II: Y15B - $62,409.27 + Full State Employee Benefits Minimum Qualifications Level I: Possession of a valid permanent Oklahoma license as a registered nurse approved by the Oklahoma Board of Nursing or Registered Nurse Licensure Compact (eNLC) Two years of professional nursing experience Level II: Possession of a valid permanent Oklahoma license as a registered nurse approved by the Oklahoma Board of Nursing or Registered Nurse Licensure Compact (eNLC) Three years of professional nursing experience Job Responsibilities Fulfill medical eligibility assessment for Title 19 nursing home admissions, State Plan Personal Care, and Advantage Waiver applications. Complete a thorough evaluation of the medical level of care eligibility for each applicant. Conduct telephone, electronic, and/or in-person home visits in the assigned area with the applicant to complete a comprehensive assessment used to identify needs and make recommendations for medical eligibility for long‑term care programs. Frequently lift up to 15 lbs. for transporting the computer. Assist in other areas as needed due to high application volume. Assist with Adult Protective Services evaluations as requested. Maintain stable and reliable home internet (not a hotspot) for telework or be required to work in the office. Knowledge, Skills, and Abilities (KSA) The KSA required includes knowledge of complex health concerns and issues; evaluating adherence to established healthcare standards and the effectiveness of health management plans/revisions; independence in planning and completing work assignments; and professional nursing knowledge in reviewing, auditing, and analyzing direct care providers’ services to ensure appropriate healthcare resources are used.
