Day shift, 5x8 hours (08:00 - 17:00)

Assignment Details

Contract has a minimum of 6 skilled home care visits per day, with potential to increase based on manager's discretion. 1-2 weekends required per contract.

Requirements

• 1 year of home health experience required
• Valid state license
• BLS certification
• Experience with assessment, documentation, patient education, safety, and various medical conditions
• Must have valid driver's license and vehicle for home visits
• COVID-19 vaccination or approved exemption form required
• Flu shot or declination required, with mandatory mask-wearing at all times

Responsibilities

• Conduct patient assessments and develop individualized home exercise programs
• Document patient progress and maintain accurate records
• Educate patients and their families on treatment plans and safety measures
• Utilize appropriate therapeutic equipment and devices
• Monitor and report any signs of physical, sexual, or psychological abuse
