Hospice Registered Nurse (RN) Case Manager – Full-Time | Vernon Hills, ILJourneyCare Hospice prioritizes manageable caseloads, strong interdisciplinary support, and giving our nurses the time they need to truly care for patients and families.We are seeking a compassionate and experienced Hospice RN Case Manager to provide patient-centered, end-of-life care in the Vernon Hills area.Schedule & TerritoryTerritory: Vernon Hills, 60061 and surrounding communities.Monday–Friday, 8:30 AM – 5:00 PM.
