Remote Patient Monitoring Specialist LPN Ma Physician Practices

Full-Time Monday - Friday.

No Weekends & No Holidays

This position is Hybrid and will require on-site training.

JOB REQUIREMENTS

EDUCATION
Minimum: Graduate of an accredited Licensed Practical Nursing (LPN) program or completion of an accredited Medical Assistant program. Current Indiana LPN license (if applicable) or Medical Assistant certification.

Preferred: Associate degree in a healthcare-related field. Experience with Remote Patient Monitoring (RPM) care coordination ambulatory care or chronic disease management programs.

EXPERIENCE
Minimum: One (1) year of experience in a physician practice ambulatory care setting care coordination program or related patient care environment. Experience using electronic medical records (EMR) and medical technology systems.

Preferred: Two to three (2-3) years of experience in chronic disease management patient outreach care coordination population health or remote patient monitoring. Experience providing patient education scheduling documentation and follow-up support. Knowledge of RPM workflows patient engagement strategies and device troubleshooting. Experience working with patients with chronic conditions including hypertension diabetes COPD and heart failure.
