Hospital Pharmacist

We're hiring an experienced inpatient Pharmacist for a 13 week contract at a long term acute care hospital in Charleston, WV . This is an excellent opportunity for an inpatient pharmacist looking to utilize strong clinical skills while working alongside a multidisciplinary healthcare team caring for medically complex patients.
Position Details- 13 week contract
- ASAP start
- Day shift
- 8 hour shifts | 5 days per week
- Competitive weekly pay
- Full time benefits such as medical, dental, vision, etc.
- Complete medication reconciliation and develop accurate prior-to-admission medication lists
- Provide discharge medication counseling to patients and families
- Verify medication orders and ensure safe, accurate medication dispensing
- Monitor drug therapy for interactions, contraindications, allergies, and appropriate dosing
- Perform pharmacokinetic dosing and monitoring
- Manage anticoagulation dosing and monitoring
- Participate in antimicrobial stewardship initiatives, including culture review and treatment recommendations
- Prepare and verify sterile and non-sterile medications
- Maintain controlled substance accountability and pharmacy inventory
- Support medication-use evaluations, quality improvement initiatives, and Pharmacy & Therapeutics activities
- Ensure compliance with state and federal pharmacy regulations, accreditation standards, and hospital policies
- Educate pharmacy staff, healthcare providers, patients, and caregivers as needed
- Active West Virginia Pharmacist license
- Hospital/inpatient pharmacy experience
- EPIC experience
- Strong clinical knowledge in medication reconciliation and inpatient pharmacy practice
- Experience with antimicrobial stewardship
- Experience with pharmacokinetic dosing and monitoring
- Experience with anticoagulation dosing and monitoring
- Strong communication and interdisciplinary collaboration skills
- Ability to work independently in a long-term acute care hospital setting
