We are seeking a Physical Therapist to support home health services within the assigned coverage area. This role involves providing high-quality patient care through evaluations, start of care visits, OASIS assessments, and follow-up/supervisory visits.

We are seeking a Physical Therapist to support home health services within the assigned coverage area. This role involves providing high-quality patient care through evaluations, start of care visits, OASIS assessments, and follow-up/supervisory visits.
Key Responsibilities:
- Perform evaluations, start of cares, OASIS visits, and return/supervisor visits
- Deliver patient-centered therapy services in a home health setting
- Meet productivity expectations (30 points)
- Travel within the assigned coverage areas
- Shift: 8:00 AM - 4:30 PM
- Weekly Hour Guarantee: 32 hours guaranteed; up to 40 hours
- Weekend/On-Call: Varies (approximately every 8 weekends)
Compensation & Benefits:
- Competitive hourly pay
- Mileage reimbursed between patient visits (from first to last patient)
- Physical Therapist (PT) degree required
- Active professional PT certification
- Associate's Degree (minimum education requirement)
- Occupational health screening required
