Our facilities function as Skilled Nursing Facilities (SNFs) that offer:

• Long-term care for residents needing ongoing skilled assistance and compassionate daily support
• Short-term, sub-acute rehabilitation for individuals recovering from surgery, illness, or injury
• Collaborative care through a multidisciplinary team approach involving nursing, therapy, and medical staff
• A varied patient demographic, including those with orthopedic, neurological, cardiopulmonary, and post-acute needs

The patient mix may differ slightly across facilities. As a therapy professional, your role will be to assist patients in achieving functional improvements and enhancing their quality of life in a supportive, patient-focused setting.

The Speech Language Pathologist (SLP) delivers direct rehabilitation services in speech therapy to patients referred by physicians, adhering to all relevant laws, regulations, and organizational standards.

• Graduation from an accredited speech therapy program (minimum MS or MA in SLP)
• ASHA certification (CCCs) OR eligibility for a 9-month Clinical Fellowship Year (CFY) to obtain ASHA certification (CCCs) and/or meet ASHA credentialing standards. Once obtained, ASHA credentialing must be maintained.
• Current registration/licensure in the applicable state, with an active license in good standing throughout employment.
• Preferred one (1) year of experience; however, new graduates are welcome to apply.
• CPR certification required upon hire or to be obtained during orientation, with ongoing maintenance throughout employment.

• Engage in community health initiatives/projects as appropriate.
• Proficient in Microsoft Word, Excel, and email communication.
• Coordinate with patients, families, and support departments to effectively address patient needs.
• Exhibit proper body mechanics at all times.
• Demonstrate the ability to make independent decisions when necessary.
• Familiarity with practices, procedures, and regulations governing operations in post-acute care facilities.
• Implement and interpret departmental programs, goals, objectives, policies, and procedures.
• Perform competently in all areas, including patient rights, safety, and sanitation.
• Maintain professional relationships with colleagues, vendors, and others.
• Ensure confidentiality of all proprietary and sensitive information.
• Understand and adhere to organizational policies, including those related to harassment and compliance.
• Exhibit integrity and professionalism by following the organization's Code of Conduct and completing required compliance training.

• Establish, evaluate, and adjust realistic, measurable, and functional goals.
• (SLP CCC only) Supervise and assess care provided by SLP CFYs.
• (SLP CFY only) Follow speech therapy treatment plans under the guidance of the supervising SLP CCC, in compliance with Federal and State regulations.
• Document care accurately and in a timely manner.
• Utilize therapy software effectively and accurately.
• Provide excellent customer service and maintain a positive attitude towards patients.
• Assist in the safe evacuation of patients when necessary.
• Demonstrate reliable and consistent attendance.
• Employ reasoning skills and sound judgment in all situations.
• Communicate effectively and collaborate within an interdisciplinary team.
• Engage in physical activities such as sitting, standing, bending, lifting, pushing, pulling, stooping, walking, reaching, and moving intermittently throughout the workday.
• Read, write, speak, and comprehend the English language.
