Seven Healthcare are seeking a skilled Physical Therapist for a travel assignment based in Grand Junction, CO. This contract-to-hire role offers an excellent opportunity for professionals with HH and HCHB experience, aiming to provide high-quality physical therapy services in a dynamic healthcare environment.

- Perform physical therapy evaluations and treatments based on physician orders/referrals.
- Complete comprehensive evaluations using appropriate assessment tools and procedures.
- Review patient records for current diagnosis, past medical history, precautions, and contraindications.
- Establish measurable goals and develop treatment plans for patient independence and function.
- Regularly re-assess treatment plan effectiveness and progress towards goals.
- Communicate patient needs and progress to the treatment team, physician, and family members.
- Consult with medical providers regarding patient evaluation, treatment, and discharge.
- Recommend and assist in securing adaptive equipment for patient function improvement.
- Maintain and submit documentation as required.
- Adhere to company policies, standards, and regulatory requirements.
- Supervise Physical Therapist Assistants and Aides in patient care activities.
- Participate in staff development programs and maintain clinical practice skills.
- Valid Physical Therapist license
- Home Health (HH) experience
- Experience with HCHB
