A short distance away from Ogema (where our facility is located) is Fargo, the Gateway to the West and a Livability Top 100 Best Place to Live, is the largest city in North Dakota. Located on the banks of the Red River, which separates it from neighboring Moorhead, MN, Fargo is jam-packed with museums, galleries, and an Art Deco theater palace in its vibrant downtown district. Outdoor activities, low taxes and strong workforce helped it rank as one of the Best Places to Live by Men's Journal. Three public colleges, including North Dakota State University, call Fargo-Moorhead home, as does the private Concordia College.

About the Facility: White Earth Indian Health Service

The White Earth Indian Health Service continues to improve its healthcare delivery model. By adopting a patient-centered medical model approach, quality comprehensive care is provided by our care teams. Our care teams are led by a provider/dentist, with the patient and their family being the focus and an integral part of the team. Experience has shown that this approach improves both patient and staff satisfaction, as well as improving health outcomes. At this facility, we provide the following services to our local population: primary care, pediatrics, specialty clinics, dentistry, and telemedicine.

Physical Therapist Responsibilities:

• Evaluate and provide rehabilitation services in accordance with his/her credentialed privileges. Types of disabilities may include, but are not limited to, post-stroke, head injury, arthritis, burns, cardiac conditions, polio, muscular dystrophy, cerebral palsy, muscle or bone disorders, including strains, sprains, post-fractures, bursitis, and scoliosis, post-surgical, and both acute and chronic pain patients.
• Develop and teach an appropriate plan of care involving the patient and family in developing goals and objectives.
• Provide therapy treatments with periodic evaluations of the program to determine the patient's status and the effectiveness of the treatment regimen.
• Document in the patient's electronic health record regarding progress while consulting with referring physicians regarding the progress of their patients.

Physical Therapist Qualifications:

• Must be a U.S. citizen.
• Graduate of an accredited physical therapy program in the United States.
• Must have an active Physical Therapy license within the 50 states with no restrictions.
• Must possess and provide documentation of continuing education.
• Must have a current BLS certification.
