Physical Therapist

The Physical Therapist is responsible for providing skilled physical therapy services to patients, including comprehensive assessments, development and implementation of individualized plans of care, and coordination with interdisciplinary teams. The role involves supervising support staff, ensuring compliance with regulatory standards, maintaining accurate documentation, and promoting rehabilitation services. The Physical Therapist actively manages patient schedules to optimize productivity and quality of care while engaging in continuous professional development and community outreach. Responsibilities:

• Monitor and amend patient visit schedules to maximize productivity and meet patient needs.
• Perform comprehensive physical therapy assessments including psychosocial, educational, and safety parameters.
• Develop and document individualized plans of care with patient-specific goals.
• Provide skilled physical therapy treatments and patient/caregiver education.
• Coordinate patient care with physicians, nurses, caregivers, rehab services, and students.
• Supervise and direct clinical activities of PTAs, PT students, and home care aides.
• Complete and coordinate documentation and paperwork to ensure compliance with regulations and support billing processes.
• Participate in in-service training and promote rehabilitation services to customers and the community.
• Conduct patient-related health assessments and procedures such as staple/suture removal and wound care.
• Communicate changes in patient care schedules promptly to relevant staff.
• Complete OASIS time points and ensure accuracy of clinical notes and documents.
• Follow agency protocols for visit documentation and billing processes.
• Assess patient needs for home care aide services and coordinate plans of care as necessary.
• Coordinate safe patient discharges and transfers with appropriate documentation.
• Report patient incidents to quality improvement specialists in a timely manner.
• Participate in peer review audits and provide/receive professional feedback.

• Licensed Physical Therapist in the state of practice.
• Knowledge of federal and state regulations, organizational policies, and APTA guidelines.
• Strong clinical assessment and treatment planning skills.
• Experience with patient care coordination and interdisciplinary collaboration.
• Proficiency in documentation, billing, and use of electronic medical records.
• Excellent communication and interpersonal skills.
• Ability to manage patient schedules and adjust to changing demands.

• Experience with home health care and OASIS documentation.
• Experience supervising PTAs, students, and home care aides.
• Participation in community outreach and marketing of rehab services.
• Knowledge of wound care and minor patient procedures.

• Must comply with state and federal guidelines and agency policies.
• Requires coordination with multiple healthcare professionals and departments.
• May involve travel to patient homes or other care sites.
• Responsibility for timely and accurate completion of regulatory documentation.
• Engagement in ongoing education and peer review processes.

• Work schedule determined by patient visit demands and agency requirements.
• May include variable shifts based on patient needs and supervisor direction.
• Timely communication with scheduling staff is essential for efficient visit management.
