Rehabilitation Case Manager - Compassionate Care Coordination

Virginia Mason Rehabilitation, part of Lifepoint Health, is seeking a PRN Case Manager to support patient care and discharge planning in the Tacoma area. You will collaborate with interdisciplinary teams, complete IPoC documentation per CMS, and participate in family conferences to ensure smooth transitions. Qualified candidates will hold a bachelor’s degree in nursing or social work, with BLS and ACLS/PALS timing requirements.
