Join us as an RN Coordinator at Geisinger’s Healthplex Woodbine to strengthen that impact. Job Duties A typical schedule is full-time, Monday through Friday, 40 hours weekly, from 8 am to 4:30 pm. At least two (2) years of RN work experience is required. Previous Med/Surg, Medical / Surgical hospital RN experience is helpful and preferred for success in the role. Qualifications Graduate from Specialty Training Program‑Nursing (Required) or Bachelor’s Degree‑Nursing (Preferred). Minimum of 2 years nursing experience. Licensed Registered Nurse (Pennsylvania) – RN State of Pennsylvania. Basic Life Support Certification (default issuing body). Knowledge of disease management, risk stratification, EPIC documentation, laboratory coordination, IV management, medication and vaccine administration. Strong communication, patient education, and interdisciplinary teamwork skills. Responsibilities Serve in an expanded nursing role in an interdisciplinary team to promote preventative patient care, offer screening services, and coordinate disease management services while providing primary clinical support. Assist clinic team with triage of in‑basket messaging and management of patient interaction with the clinic, identifying patient needs, intervention pathways, clinical guidelines, and diagnostic tests to develop individualized plans of care. Assess the healthcare, educational, and psychosocial needs of the patient and family and involve them in establishing a treatment plan based on realistic goals and interventions. Investigate healthcare options and facilitate communication among the patient, primary care provider, and other members of the healthcare team to eliminate barriers and identify interventions for treatment. Apply age‑appropriate learning principles to educate patients, family, and other health‑care members on care coordination, disease, and case‑management programs and outcomes. Serve as a resource to patients, families, and other healthcare team members regarding disease management. Implement clinical interventions based on risk stratification and evidence‑based clinical guidelines. Collaborate with primary care providers to enhance guideline adherence and promote best practice by initiating and adjusting therapies, providing follow‑up, and monitoring as needed. Coordinate laboratory and diagnostic tests for all disease‑management programs. Maintain required documentation in EPIC for all disease‑management activities. Collect and enter data and patient information according to department standards. Attend and participate in professional and nursing continuing education programs and exploit professional development opportunities. Review current literature regarding effective teaching/learning strategies and disease‑management strategies and incorporate appropriate techniques into practice. Maintain active involvement in post‑discharge transitions of care coordination. Assist leadership in the design, implementation, and evaluation of care coordination programs to support overall goals and strategic business strategies. Collect and prepare laboratory specimens and complete associated paperwork to expedite laboratory studies with maximum accuracy and efficiency. Insert, monitor, regulate, and discontinue IVs as required by provider and per scope of practice. Administer medications and vaccines, including intradermal and skin testing, under the direction of a provider and per scope of practice.
