$7,500 Sign-On/Retention Bonus for Qualified Candidates As a licensed practical nurse, you will assist in call center functions while performing duties in transitional care management and chronic disease management. You will support the physician or mid-level provider by assisting in the development, implementation, and evaluation of patient care in the office. You will be required to coordinate care with other services or outside health care providers. The LPN will serve as a liaison between the physician, the mid-level provider, and all other health care providers.
