Licensed Practical Nurse (LPN) Currently looking for 3P-11PM shift

Per Diem - $42.00 plus differential.

SIGN ON BONUS - $2500.00 (Prorated for Part Time)

QUALIFICATIONS of the Licensed Practical Nurse / LPN:

• A current unencumbered license to practice nursing in the state in which the center is located.

• Proficiency in basic computer skills including an electronic health record, PCC is a plus!

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES AND STANDARDS of the Licensed Practical Nurse / LPN:

• Demonstrates the ability to implement systems that address each resident’s needs. Having a broad scope of knowledge and experience to be able to address the needs of the medically complex as well as the mentally compromised residents.

• Demonstrates the ability to assess the health care needs of the resident, make a nursing diagnosis, transition this into an interdisciplinary plan of care utilizing all resources available. Then is able to communicate this plan to all care providers in a way that ensures continuity of care for resident.

• Demonstrates the ability to establish and maintain functional communications with staff, other disciplines, residents, families, medical staff and community.

• Treats residents, staff and facility community with respect and dignity. Treating resident and facility property and information with honesty, confidentiality, and respect.
