Medical Social Worker – Social ServicesSchedule: Weekend coverage is required. Must commit to working 3 of the following 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day.Assignment DetailsCaseload of up to 35 patients. Orientation consists of 40 hours.RequirementsMinimum of 1 year of experience as a Medical Social Worker.Must have an active compact state license.Required certification: MSW.Experience with discharge planning, care coordination, case management, and referrals to rehab/long-term care.Familiarity with documentation of care, education, and plans.Knowledge of adult/child protective services and substance use treatment options.Skill in suicide/homicide/self-harm assessment and planning.Experience working in ED, ICU, and Medical-Surgical settings.Charting system experience with Epic is preferred.ResponsibilitiesConduct assessments for patients and develop discharge plans.Provide care coordination and case management services.Document patient care and education efforts accurately.Participate in care conferences and multidisciplinary team meetings.Facilitate referrals to behavioral health services and rehabilitation programs.Assist patients and families with understanding resources and supports.Compliance / NotesSubmission of 2 references required, covering the last year. Compliance with background checks is necessary. Pre-approved time off requests must be submitted upfront. Candidates should not contact the facility directly without prior arrangement.
