Registered Nurse (RN), Mobile MDS CoordinatorCommuniCare Health Services is currently looking for a Registered Nurse to fill the key position of RN Mobile MDS Coordinator for its skilled nursing facilities throughout our Midwest Division. The successful candidate for the position must have a valid RN license in Ohio, as well as previous Long Term Care and MDS experience. Multi-facility MDS experience or Senior MDS experience preferred. The Mobile MDS Coordinator will be detail-oriented and a great team worker. He/she must be able to communicate effectively with residents, their families, and co-workers at all levels of the organization. This role will be working directly in our facilities to give addition support. This position requires travel throughout Ohio and at times to Indiana.
