Responsibilities
- Administer prescribed medications and monitor residents per individualized care plans; document changes and escalate to the supervising nurse as needed.
- Provide bedside nursing tasks within scope, including vital signs, routine assessments, and basic wound care.
- Assist residents with activities of daily living, mobility support, and supervision to promote safety and dignity.
- Maintain accurate, timely documentation in patient charts; communicate changes and collaborate with the interdisciplinary team.
- Adhere to infection prevention protocols, safety policies, and privacy regulations; ensure a safe environment for all residents.
- Collaborate with RNs, therapists, and other care team members to support care transitions and ongoing resident well-being.

Requirements
- Active Licensed Practical Nurse license (unrestricted) to practice.
- Graduate of an approved practical nursing program.
- Experience in long-term care setting preferred.
- Ability to work evening and/or overnight shifts; available for guaranteed hours.
- Strong communication, documentation, and teamwork skills.
