Physical Therapy Assistant (PTA)
A full-time Physical Therapy Assistant, (PTA), position is available in our sub-acute/skilled nursing rehab department located in Farmington.

Requirements:
- Provide comprehensive and compassionate therapy services in a sub-acute/skilled nursing facility setting
- Work collaboratively with other rehab therapists and nursing staff in a supportive team-working environment
- Possess a current Physical Therapy Assistant license or be license eligible to practice in the state employed
- Demonstrate strong computer skills with Microsoft Outlook, Word, Excel and have experience with electronic documentation
- Demonstrate good attention to detail and timeliness with documentation
- Work well with others in a team setting
- Be flexible with hours.
Good Faith Rate Range: $29/hr - $35/hr
Rehab Director / Speech Language Pathologist (ST / SLP)
A full-time REHAB DIRECTOR / Speech Language Pathologist, (ST / SLP), position is now available in our sub-acute/skilled nursing Rehab Department.

Qualifications:
- Ensure efficient and effective operations
- Plan and implement rehabilitation programs and procedures to optimize patient outcomes
- Monitor patient admissions, schedules, MDS assessment periods and data
- Provide administrative rehabilitation supervision
- Serve as a rehabilitation staff mentor
- Provide speech therapy services as needed.
- Be a licensed speech therapist or license eligible in the state where employed.
- Have experience as a treating therapist in the Skilled Nursing Facility setting
- The ideal candidate will have experience managing a SNF rehabilitation department.
