Responsibilities

• Provide comprehensive speech-language pathology services to residents in the LTC setting, including assessment, diagnosis, and treatment planning for communication and swallowing disorders.
• Develop, implement, and monitor individualized treatment plans; document progress with timely notes and updates.
• Conduct bedside and chart-based assessments; screen for dysphagia and coordinate care with the multidisciplinary team.
• Collaborate with physicians, nurses, therapists, and families; participate in care conferences and discharge planning as needed.
• Educate residents and families on therapy goals, strategies, and home exercise programs; promote continuity of care across settings.

Requirements

• Active Speech-Language Pathologist license in North Carolina or eligibility to obtain licensure.
• Experience in acute care or skilled nursing facility settings preferred.
• Strong written and verbal communication skills; proficient in documentation.
• Ability to collaborate within a multidisciplinary team and adapt to a fast-paced environment.
• Availability for a travel assignment and adherence to assignment duration and schedule.
