Licensed Practical Nurse (LPN) – Home CarePenn Home Health Care is seeking a dependable and compassionate Licensed Practical Nurse (LPN) to provide skilled nursing services to a participant in the Northeast Philadelphia (19149) area.This is a one-on-one home care assignment focused primarily on wound care, medication management, ongoing assessment, and coordination of the participant's skilled nursing needs.Responsibilities:Perform wound assessments and provide wound care as orderedAdminister and manage medications in accordance with physician orders and the plan of careMonitor the participant's condition and identify/report changesComplete accurate and timely skilled nursing documentationFollow infection-control and safety proceduresCommunicate with the RN, physician, service coordinator, and other members of the care team as appropriateProvide participant education related to medications, wound care, and the plan of careFollow all Penn Home Health Care policies and applicable state and payer requirementsQualifications:Current, unrestricted Pennsylvania LPN licenseCurrent CPR/BLS certificationPrior wound-care experience preferredExperience with medication managementHome care, home health, private-duty nursing, or community-based nursing experience preferredAbility to work independently in a home settingReliable transportationMust successfully complete all required employment screenings, clearances, and credentialing requirementsAssignment Details:Location: Northeast Philadelphia, PA 19149Patient population: AdultPrimary needs: Wound care and medication managementOne-on-one home settingSchedule/hours to be discussed during screeningJoin a growing Pennsylvania home care and home health provider committed to delivering safe, reliable, participant-centered care in the community.Apply today to learn more about this LPN assignment.Penn Home Health Care is an Equal Opportunity Employer.
