Details Home health setting covering an 8-county service area including Braxton, Gilmer, Calhoun, Nicholas, Webster, Clay, Randolph, and Pocahontas counties Patient population includes general orthopedic conditions (hips, knees), generalized weakness, strengthening needs, and cardiac rehab Typical caseload of 5-6 units per day (SOC=2, Discharge=1, Recert=1.5, Regular visit=1) Day shift schedule, 8:00am-4:30pm, Monday-Friday Self-scheduling allowed, with flexibility to arrange patient visits by area No weekend or holiday requirements Uses Epic EMR for documentation Works alongside a team of 4 PTAs Requirements Minimum 2 years of physical therapy experience required Current BLS certification required State of WV PT licensure required if applicable Open and positive attitude toward learning the Epic documentation system required Additional Information Main responsibilities include providing home health physical therapy services across an assigned multi-county territory Preference for candidates able to help cover Webster/Nicholas counties, though coverage area may be adjusted based on candidate location Department-specific orientation takes place at the Gassaway office, focused on Epic documentation and tailored to clinician needs Dress code includes navy scrubs or khaki bottoms with a navy polo Cancellation policy allows for 2 shifts per 13-week assignment American Traveler Job ID P-743446.
