Board Certified Urology - Preferred. Will accept Board Eligible providers if they are within 5 years from residency

North Dakota License - Highly Preferred

Avg callback: 1-5 hours

Duties: Clinic and Call duties

FTE: 1.0

Required Procedures: cystoscopy, cystoscopy and dilation

EMR: EPIC

Support Staff: 3 RN's, 1 CNA

Timeframe: 90 days

Requirements: completed credentialing application, ND license, DEA license, supplemental documentation, case logs
