Physical Therapist - PRN

Position Physical Therapist - PRN (Primary Location: Sub-Acute Rehab / Main Hospital, Grand Rapids, MI) Summary Assumes full responsibility for evaluating patients' need for physical therapy, plans and executes individual treatment programs in accordance with the State of Michigan licensure and PT scope of practice, and maintains treatment skills through continuing education and evidence-based research. Essential Job Responsibilities Provide patient care within scope of license or certification, as required by law and regulations. Evaluate patient condition, establish treatment goals collaboratively with patient/family and interdisciplinary team. Create written treatment plans and administer treatments accordingly. Delegate responsibilities to PT assistants and rehabilitation technicians as appropriate. Re-evaluate patient status and treatment effects; revise treatment plan and goals as needed. Evaluate medical equipment needs, order equipment, instruct patient/family and staff on use. Participate in discharge planning, communicating with patient, family, community support, and treatment team. Document patient status and progress in initial evaluation, progress notes, and discharge summary, adhering to internal/external time standards and reimbursement guidelines. Communicate with interdisciplinary team to integrate PT treatment with overall patient care; attend conferences/discussions as required. Provide patient and family education, including written instructions on PT procedures. Attend meetings and in services as designated. Follow hospital policies and procedures, including patient confidentiality and infection control. Meet or exceed program productivity expectations. Provide discipline‑specific coverage across the Mary Free Bed system as requested. Adhere to Mary Free Bed Standards of Behavior when interacting with patients, coworkers, and community members. Support program operations, including orientation and training of new staff, serving as clinical instructor for PT students, and participating in program development or research activities. Meet or exceed clinical outcome collection and performance improvement goals established by department leadership. Essential Job Qualifications Graduate of a Commission on Accreditation in Physical Therapy Education (CAPTE) program. Required licensure in the state of practice pursuant to US state laws. Valid BLS certification. Knowledge of age‑specific principles of growth and development and ability to assess patient status relative to age‑specific needs. Demonstrated ability to assess data, interpret information, and provide age‑appropriate care as per departmental policies and procedures. Physical Requirements Remain in a stationary position: None.
